Dear Futsal Player/Coach/Team Manager,

Thank you for participating in the 2005-2006 Far Post Soccer Club Futsal Leagues. In an effort to continually improve and bring you a positive league experience, we are asking that you take a few moments to answer some questions and share your comments and suggestions with us. If we are to improve, we feel it is important to get feedback from players, coaches, team managers, and parents on what worked well and what needs improvement. Please complete the form below.
